Comments: Sanitizer solution in sanitizer bucket high. Manual and mechanic warewashing sanitizers shall remain between 50 and 110ppm for chlorine. Corrected on site. Observed two spray bottle/cans of household insecticide above handwashing sink in kitchen. The use of pesticides (household) is restricted and shall be removed. All foods in freezers and walk-in coolers shall be date marked. Provide proof of Hep A vaccinations on all food handlers. Food storage containers shall be commonly identified. Observed cooking oil in dishwashing bottle with no label. All foods in plastic shopping bags in freezer and walk-in coolers shall be removed. Food shall not be contaminated by any other source. Test strips for testing sanitizing solution shall be provided. Spray nozzle hanging below flood rim in kitchen. Backflow prevention device or air gap shall be provided on plumbing fixtures as required. Corrected on site. Physical facilities shall be maintained in good repair and cleaned as often as necessary to keep them clean. Clean and organize kitchen shelves. Repair seal strip on freezer door in back of kitchen by back door. All violations need to be corrected by follow-up inspection on Jan. 10.

Employee explained that water is turned off from below the prep sink when not in use to stop the sink form leaking. The hand sink will be repaired to operate properly.
